Property Use Application

To be submitted TWO weeks prior to the event.

Please do not assume you have use of the property until you receive confirmation that your request was approved.

 

I will be responsible for picking up and returning the property requested on the dates specified. If a change needs to be made regarding these items, I will make arrangements with the church office. I agree to return all property to its original location and in the same condition as found. If damage or loss occurs during my use of the stated property, I will be responsible for any cost(s) incurred.
